Medical Insurance Information and Photography Release
All children enrolled in Vacation Bible School at Aldersgate are required to have information on file about whether or not they are covered by medical insurance. This is confidential information kept on file by the director.
Also, we take photographs and videotape the children throughout the week so we can use the pictures on our website, in our newsletter, etc. Please let us know if there is any restriction on using your child's photo for this purpose.
You may download the form to fill out and bring with you on the first day of VBS to turn in when you check in your child(ren).
